Did you know that if you are handling the affairs of a deceased Medi-Cal member, you have to provide a “Notice of Death” to the Director of DHCS? You actually must provide the “Notice of Death” within 90 days of the date of death. Plus, along with that, you must send a copy of the death certificate.
Satisfying The “Notice of Death” Requirement
It’s actually very easy to meet this “Notice of Death” requirement. All you have to do is complete and submit the online form (found here) with a copy of the death certificate. Of course, you could also mail “Notice of Death” with a copy of the death certificate to: Department of Health Care Services, Estate Recovery Section, MS 4720, P.O. Box 997425, Sacramento, CA 95899-7425.
